Childrens Case Coordination

YSSN collaborates with other agencies to support children and youth (ages 0-19, or up to 21 if still in school) and their families in York Region.

These children and youth have complex social, emotional, developmental, or physical needs that require a coordinated support plan. The goal of this service, provided in partnership with Children’s Treatment Network (CTN), is to empower families and youth to practice self-advocacy within their community.

How we can help

  • 12-18 months of service coordination and case management.
  • Brief resource support while you are waiting for coordination services, have a new diagnosis or are getting ready to transition.
  • Care plans are centered around the 6 F-words of Childhood Disability.
  • Develop a care plan with clearly outlined goals.
