Case Management

YSSN’s caring and collaborative team will work with you to identify your goals, create a plan for the services you or your family need, apply for available services, and connect you to the right community resources and supports. Our goal is to help you achieve yours!

YSSN’s case managers assist children and youth with developmental delays, as well as adults with developmental disabilities, and their families in York Region.

They help plan the services needed and connect them to community resources.

We can help connect you to:

• respite planning and camps
• special education services
• government funding resources
• health services
• children, youth and adult services

While you’re waiting to meet with a case manager, YSSN also offers Resource Days for families to answer questions, provide information and help with forms and applications.
